GSA Forms Library
The GSA Forms Library contains hundreds of electronic forms in a variety of formats including
Accessible FormNet, PDF, and FormFlow. GSA's Forms and Printing Policy Division, Office of
Governmentwide Policy, has responsibility for obtaining required approval for these forms.
The Library includes the following forms: GSA Forms are those originating within General
Services Administration. They are most often used by GSA associates, contractors and
customers.

Thrift Savings Plan (TSP) Forms
The TSP forms are only for active duty members of the Army, Navy, Air Force, Marine Corps,
Coast Guard, Public Health Service, and the National Oceanic and Atmospheric Administration
(NOAA), and members of the Ready Reserve or National Guard of those services in any pay
status.

DoD Instruction 7750.7 - DoD Forms Management Program
This Instruction supersedes DoD Instruction 5000.21 to update policies governing the DoD
Forms Management Program as required by 44 U.S.C. 21, 29, 31, 33, and 3501-3511, and
GSA Federal Information Resources Management Regulation "FIRMR," Parts 201-45.104
and 201-45.5 and implements guidance issued in 5 CFR 1320, DoD Directive 7750.5, DoD
Directive 5400.7, DoD 5400.11-R, DoD Directive 5000.11, DoD 4525.8-M, DoD Instruction
7740.3, 31 U.S.C. 3511, DoD 5025.1-M, Public Law 81-754, DoD Directive 5118.3, and DoD
Directive 7740.1.

DoD Instruction 7750.7-M - Forms Procedures Manual
This Manual is issued under the authority of DoD Instruction 7750.7, "Department of
Defense Forms Management Program," May 31, 1990.  It provided guidance for the
creation, coordination, printing, control, and distribution of DoD (DD) Forms, Standard
Forms (SF), Optional Forms (OF), or DoD Component forms; e.g., Secretary of Defense
(SD), or Departmental forms such Department of the Army (DA), their change, and the
different media in which they may be created or used.
